Include health care directives in estate plan

Estate planning can be a daunting time consuming and emotionally through the work, but it is essential for everyone.

Your succession plan should not only list that will receive your property, but must also include documents that inform your family and medical providers of this type of health care you want at the end of your life, said Eileen Saint-Pierre, Oklahoma State University Cooperative specialist personal finance expansion.

"One of the forms that you should include is the advance directive for health.This is the best way to express your wishes to your family, doctors and others about the medical care you wish to receive during the time just before your death, "said St. Pierre." There are four parts to an advance directive, a living will, an appointment of proxy health care, anatomical gifts (organ donation), and general provisions. The form is available from the Ministry of the Oklahoma Human Services (OKDHS), Aging Services Division and is available in Spanish and Vietnamese."

There are a number of guidelines that must be followed to properly complete an advance directive for health. A person must be at least 18 years and sound mind. Two witnesses are required to sign the document, but they can not be related to you or to inherit from you.

Saint-Pierre said that the person named in the advance directive can complete the form. This can not be done by a family member, guardian or durable power of attorney.

"Keep in mind that you can revoke an advance directive at any time in writing or just talk to your doctor, another health care provider or a witness," Saint-Pierre.

After completing the form, make copies for your records and family members. Give the original to your doctor to be part of your medical record.

"According to the law in Oklahoma, health care providers must assume you want nutrition and hydration to keep you alive if you do not have an advance directive on file," she said.

Another paper to consider is the Do Not Resuscitate (DNR) form.MNR informed your doctors, emergency responders and other medical providers in case of cardiac or respiratory arrest, you do not consent to the administration of CPR in any form.

This form can be signed at home, is portable and is effective immediately. Take with you to the hospital or nursing and must be honored.
